Beginning in October 2005, Moreno Valley Unified School District will conduct the California Healthy Kids Survey (CHKS) to randomly selected students in grades 5, 7, 9, and 11.  CHKS is approved and mandated by the California Department of Education for compliance with the No Child Left Behind Act (NCLB) and is designed to help school district meet requirements that promote a comprehensive school health and prevention program.

As part of student registration, parents were asked to sign permission for their child(ren) to participate in school health surveys.  If you do not wish to have your child participate in the CHKS, please contact the school principal, so that we may respect your request to exclude your child(ren) from the CHKS.

While there are sample surveys available on numerous websites, Moreno Valley Unified does not administer the entire survey.  A copy of the survey that will be used in MVUSD is available for review at the district office in the Student Services Department.  If you would like to review the survey or have additional questions, please contact Patty Rucker (951) 571-5700 ext. 17320.
